Prescribers can call in a prescription, send us an ERX or fax a prescription to our toll-free number at (844) 922-7379. Please note that CareFirst Specialty Pharmacy will only compound and dispense medication upon receipt of a valid patient-specific prescription from a US licensed prescriber.

Patients and pet owners can use the forms below to setup an account or order a prescription. For patients and pet owners to have their medication processed, we will require your prescriber to provide a prescription for the medication(s). Please have your prescriber complete and send us the form directly to us or your prescriber can contact our pharmacy toll-free at (844) 822-7379 for verbal authorization.

Pharmacy Hours: Monday-Friday 9:00am-6:00pm EST. A pharmacist is also available 24/7 for after hours emergency calls.

If you would like to call us after hours, Veterinarians and Pet Owners can leave us a confidential voicemail. We are also available for emergencies after hours.
