Pain protocols may include medications beyond traditional analgesics—such as anticonvulsants, antidepressants, antiarrhythmics, antiviral agents, anesthetics, and NMDA (N-methyl-D-aspartate) antagonists—to leverage multiple mechanisms for relief.

NMDA-receptor antagonists like ketamine and dextromethorphan can block pain transmission in dorsal horn spinal neurons, reduce nociception, and decrease tolerance to and need for opioid analgesics. [Anesth Analg 2001 Mar;92(3):739-44]

By combining medications with different mechanisms, less of each drug may be needed. Topical and transdermal options can also reduce systemic side effects and bypass first-pass metabolism, broadening what can be used in a gel base.

We can formulate topical and transdermal creams that deliver high concentrations at the application site (e.g., NSAIDs for joint pain), support systemic absorption, or enable trigger-point application for neuropathic pain.
